Independent Study Contract

SHORT TERM INDEPENDENT STUDY AGREEMENT (STIS)

SHORT TERM INDEPENDENT STUDY AGREEMENT (STIS)

If it is necessary for parents to keep their children out of school for planned absences up to 15 days per school year, the District requests that a Short Term Independent Study Agreement (STIS) be used as an alternative to classroom instruction.  An STIS allows teachers to assign work to be completed while the student is absent to help the student stay current with their classes and to excuse the absences.

The request for an STIS should be made at least 10 school days prior to the planned absence.  To request an STIS please email the following informations to Marcia Kellerman at makellerman@capousd.org:
- Student's name
- Student's grade
- Beginning and Ending Dates student will not be at school
- Date student will return to school
- Prefered phone and email contacts for parent

The STIS will be initiated through InformedK12 and an email will be sent to you prompting you to sign and forward the STIS.  The STIS will be verified and forwarded to the teacher(s) so they can assign work and provide any required support materials, prior to the start of the STIS.

All work assigned as part of the STIS is due to the teacher(s) the day the student returns to school.  Failure to submit work and complete the STIS may result in loss of future STIS opportunities.
